This agreement is entered into between PioneerRx and the pharmacy submitting this form and being referred to as the "Pharmacy", on the date that this form is submitted.

1. The pharmacy expressly authorizes PioneerRx to collect from the Pharmacy prescription and Point of Sale (POS) transaction data ("Data") and to format and transfer the collected Data to McKesson's POS Smart Reports data program.
2. The collection, aggregating, formatting, and transfer of Data shall be performed by PioneerRx.The Pharmacy is required to take no active participation in this process. Such authorizations for data collection and transfer shall continue until revoked with 90 days written notice to PioneerRx or until this program is discontinued by PioneerRx.
3. The Cost of this program is $10.00 per month with a $0.00 setup fee. PioneerRx will bill the Pharmacy under the existing billing arrangements for other products and services.Fees are subject to change on an annual basis. All fees are non-refundable.
4. To the greatest extent possible, PioneerRx disclaims all warranties, including without limitation, warranties of merchantability, fitness for a particular purpose and non-infringement. PioneerRx shall not be liable for any direct or indirect, consequential, special, exemplary or incidental damages (including but not being limited to lost profits, loss of good will or labor costs) related to the transfer of the Data. Pharmacy agreesto indemnify and hold PioneerRx harmless from any liability, expenses (including reasonable attorney's fees), costs, damages, settlements.
5. The undersigned warrants that he is fully authorized to execute this agreement
